How to apply for Medicare
If you're eligible for Medicare because of ESRD, you can enroll in Medicare Part A and
Medicare Part B by visiting your local Social Security office or by calling Social Security at
1-800-772-1213. TTY users should call 1-800-325-0778.

Note: If you're already enrolled in Medicare based on age or disability, and you're already
paying a higher Part B premium because you didn't enroll in Part B when you were first eligible,
the penalty will stop when you become eligible for Medicare based on ESRD. Call your local
Social Security office to make an appointment to re-enroll in Medicare based on ESRD.

Vocational Rehabilitation (VR) is a federal and state-funded program providing services to help
individuals with disabilities enter or return to employment. It is designed to assist individuals of
work age with physical and/or mental disabilities compete successfully with others in earning a
livelihood.

Clients must work closely with the VR counselor to create an Individualized Plan for
Employment (IPE) which will help put them on the path to successful employment.
The types of services provided are based on the needs of the individual and are designed to help
the individual to become employable. Vocational rehabilitation staff will assess needs in order to
provide appropriate services. Celiain services depend upon the individual's meeting economic
need guidelines, while other services may be provided without regard to economic need.

An individual with a disability may apply directly to Vocational Rehabilitation or may be
referred by another individual or agency. A referral may be made by contacting a Vocational
Rehabilitation office in person, by US postal, or by telephone.

  • assist with insurance and other financial entitlements
  • link you with community resources
  • order a medical id bracelet when you have a permanent dialysis access site
  • explore transportation
